Mobilized peripheral blood stem cells (PBSC) have shown effectiveness in all patients and tolerance of the preparative regimen in patients with myelofibrosis. Hyperactivation of JAK/STAT signaling in hematopoietic stem cells (HSCs) is a major cytogenetic defect in MF. Inhibition of this pathway by erythropoietin may be an appealing strategy for allogeneic transplant. Recently, experimental treatment with JAK1-STAT3 and interferon (IFN)-stimulated gene antagonists has shown promising results in a mouse model of MF. Further studies in patient-derived cells are needed to validate this approach. A potentially effective approach to target the dysfunctional microenvironment of MF in the patients who are ineligible for allogeneic SCT is to use recombinant thalidomide (a ligand for aryl hydrocarbon receptor) on a trial basis. Progress has been made in the application of mobilized PBSCs in some patients with beta-thalassemia. Recent successes in HSC transplantation for beta-thalassemia emphasize the need for continued investment in the field of SCT.
